St. Thomas Community Health Center is a Federally Qualified Health Center (FQHC), as designated by the Health Resources and Service Administration (HRSA). As such, St. Thomas CHC provides healthcare to economically and medically vulnerable populations regardless of the patient’s ability to pay or insurance status.
St. Thomas Community Health Center would like to be your medical home. We offer high-quality, affordable care. Our team of dedicated providers includes Internal Medicine Physicians, Family Medicine, Primary Care, Nurse Practitioners (NPs), Physician’s Assistants (PAs), and Pediatricians. They’re joined by specialists including Licensed Clinical Social Workers (LCSWs), Optometry, Pulmonologists, Nephrologists, Rheumatology, Adult Psychiatry and HIV Care Specialists working to address your individual health needs and ensure you receive the highest quality of care. Our goal is to help you and your family stay healthy.
After hours, there is a healthcare provider on call that may be reached by calling the clinic at 504-529-5558. The on call provider will not be able to give refills and is only to be used by established patients needing to discuss an urgent health issue that cannot wait until regular clinic hours. If you have an emergency please call 911 and go to the closest Emergency Room. Please notify us from the hospital if possible.
